Man Who Died After Falling Down Elevator Shaft at Construction Site in Haverhill, Massachusetts Has Been Identified

The accident occurred at a construction site on Essex Street

The 41-year-old man from Haverhill, Massachusetts who died after falling down an elevator shaft at a construction site in the city on Tuesday morning has been identified as Carlos Ortiz, according to the Essex County district attorney.

The accident occurred at a construction site at 98 Essex St. around 10:20 a.m., according to the Essex District Attorney's Office.

Ortiz was pronounced dead at the scene. 

Ortiz was working on the roof of a nine-story building when he fell through an opening in the roof. The opening appeared to be an elevator shaft. Ortiz was working for Progressive Roofing of Haverhill at the time of the incident.

Haverhill police, state police assigned to the district attorney's office and the Occupational Safety and Health Administration are at the scene investigating.
